Swimmer's Physical Forms

Thank you for attending the Meet and Greet with Coach Walsh and Coach Hardy. An additional note from the event, please submit your swimmer's physical and emergency contact form prior to the first day of school. It takes approximately two weeks to enter the information into the system. You can turn them into the athletic trainers at VHS. They are at this school during this month.
